iPhoneAlley details a technique to trick the iPhone into using any custom audio file as your voicemail greeting.
The greeting section does not work. Call your own number and listen to the automated voice tell you whether you have messages or not. Then wait for further instructions. It will tell you to press four for personal options and then you press three to change or listen to your previous greeting.

I called into my voicemail, and tried to change the greeting to no avail. After a few attempts, I realized that I had had an "extended absence" greeting in effect. Once I switched to a "custom" greeting, I was able to record and activate a custom greeting.

Along with having my greeting screen frozen I was also not receiving any indication that a call was coming in...no ring, no vibrate, no visual indication on the phone at all. All calls were going directly to voicemail and the greeting was some funky message...maybe it was the default although custom was checkmarked.
